All packages are delivered under a Consumer Directed Care (CDC) model.
If you require assistance or would like to know more about this process, please talk with one of our friendly Home Care staff on 6277 8800.
What is Consumer Directed Care (CDC)?
The key features of CDC packages include:
- You can control the types of care you access, how the care is delivered and who delivers it to you.
- They are offered within a restorative and re-ablement framework.
- You can set your goals such as being independent for as long as possible, staying healthy, or returning to your home after being in hospital.
- You determine how much you want to be involved in managing your package.
- Delivering ongoing and formal reviews by your provider to ensure that your package still meets your needs. Remember, if your needs change you will require re assessment by the Aged Care Assessment Team to obtain a higher level of support in your package.
- Providing you with greater transparency about how your package is funded and how those funds are spent.
If you have any questions regarding the CDC model of care, please speak with one of our Home Care Team on 6277 8800.
